For Private Medicals, Insurance Forms, Army Forms, Gun Licence Forms, other private forms needing a GP to look at.

(We do not do Passport Forms)

Please drop the forms to Reception, Please be aware that the Dr has 6 weeks to complete some of these forms and they are chargeable.

For HGV & LGV please check with the Receptionist that you will need a private appointment with the GP first, as some appointments are not always needed, the Receptionist will book you an appointment with the GP, please make sure you bring a Urine Sample and Your form and Payment when you come for your appointment.

All costs for HGV & LGV Medicals are down to the discretion of the GP.
